Below are health plan documents, alphabetically organized.
Example: If you are looking for SCAN health plan, click “P-S” tab.
If you encounter an error, click the show files arrow ‘^’ ‘v’ button located at the top left.
Tips For Using This:
By clicking on the plan name, an expandable list of documents will be available for you to preview (click on the file name) or if you want to download it, click “download”.
Please note that some plans have a lot of files so it’s best to view in list. If you prefer to view in grid, click the “4 square box” at the top right. Some health plans may have multiple pages. Be sure to check the bottom right corner!
You can also close the expandable table by clicking on the health plan name again for better view.
